Support for Early Learning and Families (SELF) has developed a form and collaborative process to strengthen transitions as children and families move from child care and preschool programs into kindergarten. Families and child care or preschool providers can complete this form together to capture information about children’s strengths, interests, and approaches toward learning. This information can be used by kindergarten teachers to build relationships with young children and better inform instruction on the first day of school. The Kindergarten Transition Summary Form includes space for information from the child, the parent and the child care representative—all three components that make up a quality education support network. Information includes basic background information, languages spoken at home, health, learning style information, and social-emotional information. The form is available at www.selfwa.org.
